As the Mental Health Diversion Case Manager you will work within a community based treatment program located in Burlington County, NJ.


Schedule: Full-time; 40 hours; Tuesday-Saturday; Wednesdays, Fridays, and Saturday 9:00am-5:30pm; Tuesdays and Thursday 11:30am-8:00pm (Additionally early morning, evenings, and weekends as needed) 


Salary: $50,000/year 


Competencies: 


The potential employee will have the ability to assess and interpret information related to population; have knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; knowledge of available treatment resources and appropriate use; Population-specific communication skills.


Responsibilities:



  • Assist mental health diversion program participants in navigating the criminal justice system.

  • Support participants in complying with court-ordered conditions and individualized treatment plans.

  • Monitor participant engagement in treatment services

  • Maintain regular contact with participants, including in-person and community-based meetings

  • Provide evening and weekend availability to complement case management services and address gaps in service accessibility.

  • Monitor medication compliance when medication is prescribed.

  • Maintain regular contact with participants to promote engagement and stability.

  • Engage with individuals in jail and other judicial facilities as needed

  • Attend interdisciplinary team meetings

  • Cooperate with all investigations, internal and externally

  • Conduct comprehensive needs assessments of participants.

  • Develop individualized service plans and updates as needed

  • Provide direct service support including transportation, coordinating treatment team

  • Maintain an individual’s clinical record (electronically) that accurately reflects all service provided on a timely basis

  • Complete Daily Activities in the Electronic Clinical Record

  • Follow documentation standards based on state and program requirements

  • Meet level of service and care guidelines identified by the contract and program

  • Provide additional documentation required for clearance into other judicial facilities

  • Work across multiple counties based on program need

  • Collaborating with the Clinical Team, Judicial staff, and other stakeholders

  • Travel throughout the state of New Jersey, and out of state travel in the tri-state area as needed

  • Facilitate linkages to community-based resources within Burlington County and surrounding areas. meeting; filing applications for services; advocacy; escort to appointment; daily living skill remediation and money management and meets Agency and funding source standards

  • Provide 24 hours/7 days a week on call crisis intervention on a rotating basis, including evenings and weekends, as needed.

Qualifications:




  • Bachelor’s degree in criminal justice, Social Work, Psychology, Counseling, Public Administration, or related field; or equivalent professional experience.



  • Valid driver’s license in good standing.
