Job Description - Mental Health Therapist - Assertive Community Treatment Team

What is an Assertive Community Treatment (ACT) team? An ACT Team is a multidisciplinary group that provides comprehensive, community-based psychiatric treatment, rehabilitation, and support to individuals with severe mental illnesses. ACT teams offer services directly in the community, including medication management, therapy, crisis intervention, and assistance with daily living activities. The goal is to reduce hospitalizations and improve the quality of life by helping individuals live independently and achieve their personal recovery goals.



 


KEY RESPONSIBILITIES




  1. Provide intervention, evaluation/assessment, individual, group and family counseling to individuals involved in the ACT Team.

  2. Develop and maintain a community referral network.

  3. Provide education and training to individuals, groups, and agencies.

  4. Maintain all regulatory, contract and documentation standards and data collection, and complete necessary documentation in a timely manner.

  5. Consistent collaboration and coordination of care between all Prairie Ridge services, including, but not limited to IHH and other services.

  6. Attend ACT Team meetings, consultations, supervision, training activities and on-call rotation.

  7. Demonstrate Proficiency in clinical practices identified as best practice by the agency demonstrated through supervision and direct observation.

  8. Develop and maintain a working knowledge of all documentation and reporting systems to include ISmart, Accreditation Now, MyOutcomes, and Electronic Health Record.

  9. Demonstrate understanding and adherence to all CARF policies and procedures.

  10. Provide crisis intervention and assessments for clients seen on an emergency basis.

  11. Provide mental health consultation to other agencies or provider’s assisting ACT Team patients.

  12. Provide Family education and support as outlined in the client’s treatment plan.

  13. Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism, and objectivity. 

  14. Maintain all HIPAA and 42 CFR confidentiality requirements and regulations as outlined in CARF policies and procedures.

  15. Comply with agency policies, procedures and regulatory standards.

  16. All other duties as assigned.




    QUALIFICATIONS and USEFUL SKILLS/KNOWLEDGE REQUIRED FOR THIS POSITION




    1. A minimum of a master’s degree in behavioral healthcare and a license as a LMFT, LMHC, LMSW, LISW, or psychologist. Temporary licenses will be considered if the individual is completing supervision hours toward full licensure.

    3. Knowledge of the principles and methods of therapy including training in evidence-based practices.

    4. Knowledge of psychotropic drugs concerning their use, misuse, desirable and undesirable effects and research material available on these drugs.

    5. Considerable knowledge of mental health services and available resources.

    6. Ability to establish and maintain good working relationships with consumers, families, hospital staff and community mental health center staff.

    7. Willing to work in non-traditional and in-home settings.

    8. Ability to work a flexible schedule, including evenings and/or weekends if necessary.

    9. Travel may be required as this is a community-based position.

    10. Supportive of the mission of Prairie Ridge and the patients we serve.

    11. Proficiency in oral and written communication skills, as well as documentation requirements.

    12. Technology skills allowing for proficiency in use of electronic health record.

    13. While performing the duties of the job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools, or controls; and talk or hear. The employee frequently is required to walk and stoop, kneel, crouch, or crawl.  The employee is often required to stand, reach with hands and arms, and climb or balance.

    14. The employee must infrequently lift and/or move up to 10 pounds and occasionally lift and/or more up to 35 pounds.  Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.
