Codman Square Health Center is a Federally Qualified Health Center serving a diverse community in Dorchester, MA, and surrounding neighborhoods. CSHC offers a full continuum of services such as primary care, urgent care, behavioral health, care management, nutrition, family planning, optometry, dentistry, laboratory, pharmacy, radiology, and medical specialties including HIV care, substance use treatment, Hepatitis C, OB-GYN, Nephrology, and Cardiology alongside an on-site STI clinic.
CSHC is a teaching institution with academic affiliations at Boston University School of Medicine and Boston Medical Center, and a member of the Boston Accountable Care Organization. Our mission is to be our community's first choice for comprehensive, holistic, and integrated services, and to empower individuals to lead healthy lives and build thriving communities.
The Mobile Health Unit represents the next chapter of that mission: meeting patients where they are, closing gaps in access, and advancing health equity at the neighborhood level.
Position Summary
Codman Square Health Center (CSHC) is launching its Mobile Health Unit (MHU), a strategic initiative, to bring high-quality, team-based primary care directly into the communities we serve. The MHU extends CSHC's mission into Dorchester, Mattapan, Hyde Park, Roslindale, Randolph, and surrounding neighborhoods, reaching patients where barriers to clinic-based care are greatest.
We are seeking a skilled, mission-driven clinician — Nurse Practitioner or Physician (MD/APP) to serve as a primary care provider on the Mobile Health Unit. This is a unique opportunity to practice community-centered medicine at the intersection of primary care, preventive health, and health equity, delivering longitudinal care alongside walk-in services in community settings including residential facilities, faith-based sites, and neighborhood anchors.
This role is central to CSHC's growth strategy: expanding our patient panel, improving HEDIS quality metrics, and establishing CSHC's permanent presence in health-underserved communities. The clinician will operate as an extension of the CSHC care team, with full Epic EHR connectivity, integrated behavioral health and care management support, and direct clinical supervision aligned with Massachusetts collaborative practice standards.
