Mobile Physician Position SNF or Primary Care in San Diego, California

Position: Full-Time OR Part Time Primary Care Physician

Job Type: Full-Time Schedule: Monday to Friday (No Weekends) OR Part-Time (No Weekends), Flexible Schedule

Location: San Diego Area

Our mission is to provide high-quality and compassionate care for seniors in the comfort of their homes or senior living communities. As a primary care provider with Aligned Geriatrics you will play a crucial role in improving the lives of older adults by delivering personalized healthcare services.

Physician Responsibilities:

• Conduct comprehensive primary care visits in patients' homes or senior living communities
• Collaborate with a multidisciplinary team to ensure holistic and patient-centered care
• Provide preventive care, manage chronic conditions, and address acute issues in a timely manner
• Collaborate with and coordinate care amongst various community partners to ensure continuity of care
• Determine and initiate specialty referrals when appropriate
• Order and interpret lab and imaging testing
• Prescribe medications including controlled substances when appropriate
• Build meaningful relationships with patients and their families to promote trust and understanding
• Collaborate with staff at various senior living communities to ensure timely and coordinated patient care

Qualifications:

• Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
• Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred)
• Active and unrestricted California medical license
• Experience in Geriatric Medicine is a plus
• Strong communication and interpersonal skills
• Experience with Epic (EHR) is a plus
• Experience with Microsoft Office is a plus
• Strong time management and organizational skills
• Comfortable working independently and in team based environments
• Compassionate bedside manner with a commitment to providing patient centered care.
