General Responsibilities

  1. Conducts initial visits with clients in multiple counties, completes annual redetermination of services and serves clients in absence of volunteers.
  2. Other duties as assigned within the guidelines of this position.

Essential Functions

  1. Responsible for follow through on client referrals, completing assessments and monitoring volunteer/client placements.
  2. Responsible for following documentation requirements and timeframes as outlined in program policy.
  3. Attends initial meetings with volunteer and clients (match meet) and provides orientation to clients regarding the program, generally in the client’s home.
  4. Ensures that all volunteers and clients follow established program policies and procedures.
  5. Informs Program Manager of any challenges, problems, or concerns in a timely manner.
  6. Maintains program records to ensure compliance with agency, funding source and accreditation standards; ensures program data integrity is maintained and monitored.
  7. Represents the agency and provides information about and promotes program through active participation with community agencies/organizations; maintains cooperative working relationships with a variety of community organizations and agencies.
  8. Assists in the formal and regular recognition of volunteers, organizations and individuals who have contributed support to the Program.
  9. Accurately maintains appropriate computer database, client and volunteer file records.
  10. Maintains awareness of current information on programs and services available to older adults and persons with disabilities.
  11. Attend staff meetings, conferences and training programs as recommended by supervisor.
  12. Develops and maintains strong working relationship with referring agencies to as to increase our outreach system and public awareness.

Requirements

  1. A Bachelor’s degree is required for this position with preference in areas of human service related or business field or three (3) years of relevant experience. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. Minimum of one (1) year of experience in human services, volunteer management, banking or similar environment; experience with older adults or disability services is preferred.
  3. Demonstrated ability to work effectively and constructively with persons from all racial, diverse cultures, family systems, and social backgrounds.
  4. Experience working with older adults or persons with disabilities is preferred.
  5. Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
  6. Strong verbal and written communications skills.
  7. Passage of complete background clearance, and any other required testing or clearances.
  8. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
