Medical Social Worker MSW (PRN)

What You Will Do

• Complete social work assessments in the home
• Identify psychosocial, financial, and environmental needs
• Support care planning and discharge coordination
• Connect patients and families to community resources
• Provide counseling and emotional support
• Collaborate with nurses, physicians, and care teams
• Document visits and updates in a timely manner

What You Need

• Master’s Degree in Social Work (MSW)
• Active state social work license
• 1+ year healthcare or home health experience preferred
• Strong communication and organization skills
• Reliable transportation and willingness to travel (50%)

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
