The New Patient Navigator serves as the first point of contact for new patients. This role ensures a smooth onboarding process by helping patients understand their care options, scheduling initial appointments, assisting with paperwork, and removing any barriers to care.

Key Responsibilities:

  • Collect and verify patient information, insurance details, and health history.
  • Schedule initial appointments and coordinate with office and other departments.
  • Provide patients with clear information about our services, providers, and policies.
  • Educate patients on what to expect as a Community Health Care patient.
  • Act as a liaison between patients, providers, and administrative staff.
  • Identify and address potential barriers to care (e.g., transportation, language, forms).
  • Maintain accurate documentation and patient records.

Qualifications:

  • High school diploma or equivalent required; associate or bachelor’s degree preferred.
  • Experience in a healthcare or customer service setting strongly preferred.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to multitask in a fast-paced environment while maintaining attention to detail.
  • Empathetic, patient-focused, and culturally sensitive.
  • Proficient with computers and electronic health records (EHR) systems.
