M-F Clinic ... no weekends ... no holidays ... no call

Hospital heart failure patient rounds

Clinic staff: 2 RNs, 2 LPNs, 1 receptionist and 1 cardiologist

Charting: EMR and paper, converting to EPIC in August 2015

APNP license 

Have a Wisconsin license or ability to obtain one 

2-5 years experience preferred, but not required

3 years of experience working with CHF and anti-coagulation patients in a clinic setting.
