PATIENT INFORMATION

• Patient Demographics: All ages

• Phone Consults: As clinically appropriate

• Case Mix: Primary care and family medicine

FACILITY INFORMATION

• Office Equipment Available: Standard primary care clinic equipment

• Support Staff Available: Clinic staff

• Reason for Coverage: Provider coverage need

COMPENSATION & BENEFITS

• Travel, Lodging, and Malpractice Insurance: Covered

REQUIRED PROCEDURES

• Joint injections

• Toenail removals

• Lesion removals

JOB REQUIREMENTS

• Licenses: Active Colorado NP or PA license

• Board: National certification as Nurse Practitioner or Physician Assistant

• DEA: As required for primary care practice

• Certifications: As required by Lincoln Health credentialing

• Experience: Primary care or family medicine experience

• Other Qualifications: Comfort performing minor in -office procedures

DUTIES & RESPONSIBILITIES

• Provide comprehensive primary care services

• Diagnose and treat acute and chronic conditions

• Perform minor in -office procedures as required

• Collaborate with clinic staff to ensure efficient patient care

• Document patient encounters accurately and timely
