Job Description - Nurse Care Navigator


  

We are seeking a licensed, experienced Nurse Care Navigator to help support our existing team in New Haven County. This is part-time, per diem work. Hours fluctuate and can largely be set by the Nurse Care Navigator. 

Candidates must have at least 10 years' experience working with older adults and possess a minimum of a bachelor's degree and applicable certifications/licenses. They must be able to assess the needs of older adults and provide thoughtful care planning advice and strategies. Strong advocacy skills and the ability to work within family systems are necessary and upholding our mission to improve the quality of life of all clients.  

· Direct experience with older adults is REQUIRED; experience with individuals with memory impairment is a plus. Patience, flexibility, compassion, organization, collaboration, and timeliness are all keys to performing this work. 

· The hourly rate is commensurate with experience. 

· Sage Solutions is a subsidiary of Connecticut Community Care based in Bristol, CT. 


Requirements

Education:

· Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field required. LPN or RN required. 

Experience:

· Must have a minimum of 10 years of experience in health care or social services or related field plus experience working with older adults.

· Knowledge and understanding of social, health, and economic factors that contribute to health and wellbeing outcomes; knowledge and skill in interviewing and assessment (social and health) techniques. 

· Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care.

· Strong written, verbal and computer skills required. 

· Ability to maintain confidentiality of resident, company, and staff information.

Other:

· Reliable transportation, valid driver’s license, and current automobile insurance.

While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands or fingers, handle or feel objects, tools, or controls. The employee is occasionally required to stand, walk, sit, and reach with hands and arms. The employee must occasionally lift and/or move up to 35 pounds. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus. The noise level in the work environment is usually low to moderate.
