Are you a nurse passionate about empowering individuals with intellectual and developmental disabilities (I/DD)? At PASCO, we’re seeking a skilled and compassionate I/DD Nurse Coordinator to join our mission-driven team and help individuals thrive in their communities.

In this role, you’ll provide clinical case management and health oversight for individuals with I/DD, guiding personalized care plans and supporting Direct Service Providers. You’ll play a key part in maintaining quality care, offering training, delegating tasks in alignment with Board of Nursing standards, and advocating for those we serve.

• Deliver skilled nursing, care coordination, and clinical oversight

• Develop and update individualized care plans based on client needs

• Train and support Direct Service Providers on medical tasks and best practices

• Complete nursing delegation and ensure regulatory compliance

• Collaborate with families, providers, and interdisciplinary teams

• Manage clinical documentation and medication records

Deliver skilled nursing, care coordination, and clinical oversight

Complete nursing delegation and ensure regulatory compliance

Collaborate with families, providers, and interdisciplinary teams

• Colorado RN license (required)

• BSN and CPR certification

• 1+ year acute care experience; I/DD or home health a plus

• Strong communication, documentation, and teaching skills

• Bilingual in Spanish a plus

• Hybrid: Tu/Th in-office (Lakewood), M/W/F home visits

• Territory: North of I-70 — includes Broomfield, Longmont, Greeley, Thornton, etc.

• $36–$42/hour + mileage

• $5,000 sign-on bonus
