Summary: The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors (NHV) are pivotal in transforming the lives of vulnerable babies, mothers and families by implementing and maintaining the fidelity of the NFP model. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP and of Catholic Charities. www.nursefamilypartnership.org
Position Responsibilities:
- *Plan and provide in-home and, and/or telehealth visits (upon approval) to women and their families eligible for the NFP Program in accordance with NFP Model Elements and NFP Visit-to-Visit guidelines in accordance with client goals and NFP outcomes.
- *Adheres to nursing process and the NFP model of home visitation.
- *Work towards retention strategies to carry a caseload of 25 to 30 clients.
- *Maintains up-to-date case records within data systems provided within 2 business days and hard file within 5 business days.
- *Documents appropriately and provides information and documentation requested to support staff for timely and accurate data input to the NFP performance management system.
- *Completes all required NFP education, training, and professional development associated with service delivery and competency requirements to develops therapeutic relationships, utilizes concepts of reflection, active
- *Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains. Formulates nursing diagnosis based on nursing assessment and client goals.
- *Assists women and their families in establishing goals and evaluates client progress toward stated goals and NFP outcomes by providing education, support and referral resources to contribute to the betterment of women and their families, and to assist them in reaching their targeted goals.
- Accurately assesses own learning needs and develops strategies to meet them. Motivated to utilize computer for distance learning.
- Consults and collaborates with other professionals involved in providing services to women and families.
- Schedules joint home visits with NFP Nurse Supervisor every 3-4 months.
- Attends and participate in bi-weekly case conferences and team meetings.
- As requested/required, participates in community coalition in support of NFP, promotes public awareness of NFP, and develops and maintains community relationships to promote program growth and success.
- Maintains clear, effective, open, honest, and consistent communication with families, colleagues and stakeholders.
- Represents, understands and supports colleagues in the NFP vision, mission and model.
- Assists in creating a positive work environment that promotes productivity, mentoring, teamwork, cooperation, and recognizing the accomplishments of team members.
- Stays informed of current health care developments to provide safe, quality nurse home visiting services.
- Actively works to understand and address personal reactions and biases, welcomes and responds appropriately to feedback.
- Complete all necessary agency required professional development training in a timely manner.
- Other duties as assigned by the Program Director/Nurse Supervisor.

Minimum Qualifications:
- Must be at least 21 years old.
- Education
- Bachelor’s degree in nursing- required
- Experience
- Minimum of 1 year of experience in nursing field; experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred.
- License and Credentials
- Current Compact or State of Texas RN license (in good standing)
- Current CPR for Healthcare
- Reliable transportation
- Valid driver license and clean driving record
- Valid vehicle liability insurance
- Bilingual in Spanish and English- Preferred
Minimum Knowledge and Skills:
· Minimum of 1 years of experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred.
· Proficiency with computer software to include Microsoft Office 365.
· A solid grasp of community and maternal-child health.
· Ability to work in a culturally sensitive manner with people of various backgrounds
· Ability to build strong relationships
· Ability to maintain appropriate ethical boundaries and professional relationships
· Ability to effectively communicate with team and Nurse Supervisor
· Must be flexible and adaptable and be comfortable scheduling and rescheduling a day’s activities based on multiple factors.
· Must be detail oriented, organized, self-motivated, work well independently and on a team.
· Must have good written and verbal skills;
· Must have good critical thinking and problem-solving skills.
· Must be able to practice with a in a trauma informed care approach
· Must be willing to self-educate on health topics that may affect families.
