Nurse Practitioner

As we breathe new life into our palliative care program, Nurse Practitioners are at the center—building lasting relationships that carry patients and families from early conversations through life’s most sacred transitions. This work is rooted in compassion and designed to meet people where they are, long before hospice begins.

As part of a collaborative, interdisciplinary team, you’ll be supported in the kind of care you went into healthcare to provide. With time to listen, space to guide, and the ability to stay present across a patient’s journey, you’ll help families navigate the emotional complexity of serious illness. You’ll hold the paradox between planning and hoping—bringing clarity, comfort, and connection to moments that matter most.

Nurse Practitioner (NP) Qualifications:

• 1+  years of Nurse Practitioner experience in an acute care environment.
• Master of Science in Nursing - Family Nurse Practitioner.
• Active, unencumbered state NP license.
• Home Health, Palliative Care, or Hospice Experience preferred.
• Current driver's license and auto liability coverage.
• Detailed assessment skills in this role require recertification for Hospice patients.
• Interest in a community-based role that requires travel between patient visits.
• Drug Enforcement Agency (DEA) number required.
• National Provider Identifier (NPI) Required.

Nurse Practitioner (NP) Schedule & Benefits:

• Schedule: Monday to Friday, 8:30 AM - 5:00 PM with no on-call required
