Key Responsibilities:
· Conduct in-home visits, health assessments, and care planning for members with acute and chronic conditions.

· Collaborate with other healthcare providers to deliver high-quality, patient-centered care.

Requirements:

· Bachelor’s in Nursing with Vermont APRN license (Master’s preferred).

· Board certification for Nurse Practitioners and DEA licensure.

· Valid driver’s license and proof of insurance.
