The Certified Nurse Practitioner (CNP) will provide diagnosis, treatment, consultation, and follow up care of patients under the direct supervision of the Nephrologist and Director of Physician Services. The CNP will effectively identify, and evaluate, while administering quality care for their patients; and will communicate with the medical staff on issues concerning consultation, collaboration, and referrals. Incumbent is responsible for medical diagnosis and the treatment of critical, prolonged, and long-term healthcare issues and will also participate in educating patients and families on their preventive care, medical issues, and use of prescribed medications and treatment. They will also maintain accurate and legible medical records, while documenting all medical evaluations, diagnosis, procedures, treatment, prognosis, referrals, education, and consultations that are consistent with state regulatory standards.
EDUCATION, EXPERIENCE TRAINING AND LICENSES/REGISTRATIONS: RN, Ohio Licensure required. Master's degree in nursing with advance certificate of authority Excellent clinical and leadership skills with a commitment to participation in a patient focused environment. Dialysis experience preferred 1 year of hospital experience preferred
