  Complete assessments, create care plans, and coordinate with teams

  Close care gaps like HEDIS and STARs

  Handle prior auths, med reviews, and chronic care

  
  


• 
  

  Keep communication clear with families and facility teams

  
  

• 
  

  Active NP license in South Carolina + national certification

  
  


• 
  

  Eligible to bill Medicare

  
  


• 
  

  6+ months working as an NP (bonus if in geriatrics or palliative)

  
  


• 
  

  Solid background in chronic conditions and nursing home care

  
  


• 
  

  Good communicator, comfortable with tech
