At EasyHealth, we partner with leading health plans to deliver in-home Annual Wellness Visits (AWVs), bringing preventive care directly to members in their homes. This role is focused on assessment-based care, no prescribing, no call, and no weekends, allowing you to concentrate on high-impact patient engagement.

What you’ll do:

• Conduct in-home health assessments (history, vitals, screenings, labs)
• Identify care gaps and support preventive care initiatives
• Document visits in our EMR and coordinate follow-up care when red flags are identified
• Educate members

What we’re looking for:

• Active Oregon NP/APRN license to conduct visits in home
• Medicare/Medicaid enrollment (or willingness to enroll)
• Board Certification
• Comfort working independently in a mobile care model
• Reliable transportation
