Nurse Practitioner

 Nurse Practitioner – School-Based Health Center Family Care Health Centers | St. Louis, Missouri Family Care Health Centers (FCHC), a mission-driven Federally Qualified Health Center serving the St. Louis region, is seeking an experienced Nurse Practitioner to provide comprehensive healthcare services within our growing School-Based Health Center Program. We are looking for a Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP) with a strong foundation in adolescent medicine and a passion for improving the health and well-being of youth in underserved communities. Position Highlights

• Deliver preventive, acute, and chronic disease management services to adolescents and young adults.
• Address the unique physical, emotional, behavioral, and social health needs of adolescents.
• Collaborate closely with school staff, behavioral health professionals, care teams, and families.

• Work within an integrated care model that includes medical, behavioral health, and community support services.

Qualifications

• Current Missouri Nurse Practitioner license.
• Board certification as a Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP).
• Demonstrated experience working with adolescents and young adults.
• Knowledge of adolescent preventive care, behavioral health screening, reproductive health, sports physicals, school health requirements, and chronic disease management.
• Commitment to serving diverse and underserved populations.

Preferred Experience

• School-based health, pediatric, adolescent medicine, family medicine, community health, or FQHC experience.
• Experience collaborating with schools, community agencies, and multidisciplinary care teams.
