Nurse Practitioner

A fast-growing long-term care organization is seeking a
compassionate and dedicated Nurse Practitioner to join its
team at a nursing home in Edison,NJ. This is an
excellent opportunity for an experienced provider who is passionate about
delivering high-quality primary care to a geriatric patient population in a
supportive and collaborative environment.

Responsibilities:

• Provide
  comprehensive primary care services to nursing home residents
  
• Perform
  new admission evaluations and ongoing follow-up visits
  
• Monitor,
  assess, and manage acute and chronic medical conditions
  
• Collaborate
  with interdisciplinary care teams to ensure optimal patient outcomes
  
• Document
  patient encounters thoroughly and accurately in the medical record
  
• Support
  continuity of care and contribute to overall quality of care within the
  facility
  

Requirements:

• Active Nurse
  Practitioner license in the state of New York
  
• Active DEA
  license
  
• Minimum 6
  months of experience working as a Nurse Practitioner
  
• Prior nursing
  home or long-term care experience strongly preferred
  

Compensation & Benefits:

• Highly
  competitive salary: $150,000–$155,000 annually
