Certified Registered Nurse Practitioner

This is a Full-time position; 5 days a week with an occasional Saturday, 4-hour shift.  No holidays!

Responsibilities:

• Working with Kendal~Crosslands Communities primary physicians within a Collaborative Practice Agreement to provide medical on-site care and services for residents in our community.


• Identifying and managing chronic and acute health conditions by requesting laboratory and/or x-ray studies, initiating treatment and/or managing medication regimens with appropriate follow-up.


• Responding to emergencies and administering first aid to residents by evaluating injuries and arranging further treatment if necessary.


• Participating in planning for residents’ needs as they transition through levels of care.


• Collaborating with Director of Resident Care on any present or anticipated problems.


• Performing medical procedures as necessary.


• Adhering to Pharmacy protocols and procedures including e-prescribing.


• Administering treatments, injections and medications to residents as prescribed/necessary.


• Communicating lab and test results to residents and physicians.


• Managing INR Lab results and follow up as necessary.


• Providing wellness screenings.


• Initiating requests for consultations or referrals.


• Participating in Community Quality Assurance, Pharmacy and Infection Control Committees as requested by Director.


• Arranging and establishing orders for transfer to the Health Center or hospital and assisting with discharges from hospital back to Independent Living and or to Skilled Nursing.
