Job Description - Nurse Practitioner
Summary: Provides nursing care for patients under the direction of physician. Follows established nursing standards, procedures and practices and gives patient care direction to nursing and other staff members. Manages and performs direct patient care within the scope of the Registered Nurse Practice Act based on the nursing process. Plan, coordinate, oversee and execute an individualized, holistic plan of care regarding hospice certification of assigned patients.
Qualifications:
Graduate of an accredited school of nursing with current Texas license.
Bachelor’s degree in nursing with specialist degree as Nurse Practitioner.
Currently licensed as Nurse Practitioner in the State of Texas.
Current certification in basic life support.
Excellent administrative skills with ability to supervise staff and coordinate services.
Excellent interpersonal and communication skills, both oral and written.
Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
Ability to function as part of interdisciplinary team.
Ability to travel to multi-work locations as assigned.
Knowledge of applicable State/Federal laws governing hospice licensure.
Must have reliable vehicle, valid Texas Driver’s License and Liability insurance.
PC experience preferred.
Duties/Responsibilities:
Perform direct patient care based on the nursing process and within the scope of the Registered Nurse Practice Act.
Identify nursing problems, psychosocial, spiritual and emotional issues as they relate to the individual needs of the patient and family.
Perform face-to-face encounters and share clinical findings with hospice physician.
Assist patient/family with acceptance, cooperation and response to the hospice concept.
Report any significant changes in patient’s condition to the attending Physician and/or Patient Care Manage and request change orders as needed.
Maintain required CEUs.
Follow established computer documentation and organizational processes as outlined.
Instruct clients on appropriate techniques for care of the patient and documents.
Assist in providing service that fosters the organization’s mission, values, vision and goals.
Participate in training programs for community and area activities related to hospice philosophy including speaking to outside organizations as needed.
Understands and adheres to organizational privacy practices and compliance programs.
Any other duties as assigned by management.
