Job Summary
Assist and collaborate with the Endocrinology services/providers in practice and future programs. NP will provide direct care to adult patients living with diabetes, including medical management and patient education. Will collaborate with clinical services interdisciplinary teams to teach in a patient education program for Diabetes. Associates may be required to work at multiple locations (Inpatient and Outpatient), including clinics and hospital departments, at Mount Vernon Hospital. The role blends clinical expertise, education, and coordination to improve outcomes and quality of life.

Responsibilities:

•    Assessment & Diagnosis: Take detailed patient histories, perform physical exams, and interpret medical records to assess health status and diagnose diabetes or related conditions. Play a vital role in educating patients, managing their symptoms, and improving their quality of life.

•    Treatment & Management: Prescribe and administer medications, insulin regimens, and other therapies; order and interpret lab tests and diagnostic results.
•    Care Planning: Develop, implement, and evaluate individualized care plans addressing medical, behavioral, and lifestyle needs.
•    Monitoring: Track blood glucose levels, perform a comprehensive physical exam, including the foot, and initiate referral to ophthalmology for eye exams to prevent complications, and monitor for signs of diabetic emergencies.
•    Medication Management: Manage insulin subcutaneous and infusion, and other diabetes-specific treatments.
•    Assessing patients’ health status, including conducting physical examinations and interpreting medical history
•    Diagnosing and treating diabetes and its associated health conditions
•    Developing, implementing, and evaluating individualized patient care plans
•    Prescribing and administering insulin and other diabetes treatments
•    Educating and counseling patients and their families on diabetes self-care management, including lifestyle changes and self-care measures
•    Keeping up-to-date with the latest diabetes research, treatments, and management strategies
•    Documenting patient care and maintaining accurate medical records
•    Advocating for patients to ensure they receive appropriate health resources and services
 


Qualifications

•    Insulin pump training
•    Ability to manage multiple priorities and adapt to changing clinical needs
•    Professional Development: Stay current with diabetes research, treatment modalities, and best practices.
•    Maintain certifications such as NP licensure, BLS/ACLS, and Certified Diabetes Care and Education Specialist (CDCES) credential upon hire and or within 90 days of hire.
•     Bachelor's Degree required in Nursing with specialty NP certification
•    Master’s degree in nursing from an accredited institution.
•    Current licensure as a Registered Nurse and Nurse Practitioner in the state of practice.
•    Board certification in Family Practice, Adult Practice, or Geriatrics.
•    Minimum 2 years’ experience as an NP, diabetes experience required, with the ability to manage the use of insulin pumps, continuous glucose sensors, and related diabetes technology. 
•    Current NY State RN License as well as current NP specialty certification and registration. 
•    Bilingual (Spanish/English) a plus.
•    Strong leadership and organizational skills
•    Excellent communication and interpersonal abilities
•    Proficiency in healthcare regulations and standards
