Healthcare United is seeking a Nurse Practitioner for a full-time role focused on Geriatric Medicine in the Bolton Area, Connecticut.

Deliver longitudinal Geriatric care in patients’ homes and assisted living settings with multidisciplinary support.

What You Can Expect

• Assess and manage older adults in home and assisted living environments.

Requirements

• Family, Adult, or Adult/Gerontology Nurse Practitioner training.
  
• Unrestricted Connecticut license or eligibility.
  
• At least two years of postgraduate NP experience required.
  
• The two years of experience must be in Geriatric Medicine or a related field.
  
• Current CV required.
