Job Summary

Nurse Practitioner, MS AMB - MNA | BWH Transplant | Heart Failure | 30 HOURS | DAYS


GENERAL SUMMARY:
The Nurse Practitioner (NP) is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function as an independent licensed provider. The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care.

The Nurse Practitioner works in collaboration with the Heart Failure physicians and nursing staff and the Center for Advanced Heart Disease faculty of cardiologists and is responsible to the BWH Heart Failure program. The Nurse Practitioner participates with coordination and transition of care of the patient across all phases of the hospital, post discharge, and longitudinal heart failure setting, and will work closely with all members of the cardiovascular care team and primary care providers to deliver the best possible comprehensive care to cardiovascular patients.

This position has the unique opportunity to serve the Center for Advanced Heart Disease Community Outreach Program and will require travel to practices located in: Foxboro, Westwood, Faulkner site in Jamaica Plain, Main BWH Campus Boston, and other sites as needed. Essential Functions
A. Patient Care Organization
1. Provides direct care, counseling, and teaching to a select patient population.
2. Performs physical examinations and provides preventive health measures appropriate to patient needs.
3. Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients’ clinical problems and health care needs.
4. Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings.
5. Performs laboratory tests appropriate for specific patient population.
6. Orders medications and prescriptions according to organizational and regulatory policies and procedures.
7. Records pertinent patient care findings in the medical record.
8. Sets priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment
9. Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals.
10. Incorporates the concepts of health maintenance, prevention, and promotion into daily practice through patient education and counseling.
11. Demonstrates awareness of community resources and assists staff, patients, and families to effectively utilize them.
B. Quality of Care
1. Assists in the development, implementation and revision of nurse practitioner guidelines and monitoring requirements for prescription writing in collaboration with their designated physician, Nursing Manager or Nursing Director.
2. Evaluates outcomes of own patients’ care and makes necessary revisions in care plans.
3. Seeks and provides consultative services as required.
4. Participates in departmental and organization quality improvement program.
5. Participates in all program and transplant QAPI processes as appropriate and directed by the Nurse Manager or Nurse Director
C. Coordination
1. Facilitates continuity of care and transition of care between inpatient, post-acute care settings, and longitudinal care via multiple platforms including but not limited to clinic evaluations, telephone triage, conference calls, and medical record review.
2. Demonstrates appropriate collaborative and consultative relationships with other providers within the institution and community.
3. Serves on Department and/or Hospital Committees and assumes responsibility for disseminating and communicating information to staff.
4. Communicates with patients, referring providers and other involved parties as appropriate to relay results of evaluation and plan of care.
5. Participates in outreach efforts as directed by program leadership. This includes but is not limited to hosting a clinic at various locations including but not limited to: Foxboro, Westwood, Faulkner site in Jamaica Plain, Main BWH Campus Boston, and other sites as needed.
D. Budget
1. Participates in setting productivity metrics and in cost effective management of personnel, time, supplies, and equipment in his/her area.
2. Collaborates with the Nursing Director/Practice Manager/Executive Nursing Director in the recommendation of appropriate staffing patterns for their clinical areas.
E. Personnel Management
1. Participates in selection process of new employees in group or team practice as requested.
2. Provides input into evaluation of other staff as appropriate.
F. Education
1. Acts as preceptor for nursing students and staff as requested.
2. Acts as an educational resource for other trainees including but not limited to medical students, residents and fellows, as well as inpatient nursing and all members of the multidisciplinary transplant team.
3. Participates in orientation of new staff.
4. Participates in Departmental and organization continuing education programs.
5. Identifies own learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature.
6. Educates staff and/or professional groups through lectures, papers, publications, and educational materials.
G. Planning
1. Develops annual objectives and reviews with Nursing Director and/or Nurse Manager and collaborating physician at time of performance review and at appropriate intervals. Completes annual credentialing process through the Nursing Department credentialing program.
2. Assists in determining, evaluating, and/or implementing clinical area goals and objectives.
3. Maintains interdisciplinary approach for planning and communicating information for patient care and operation of practice.
4. Maintains current knowledge of legislation that may affect the health care delivery system.
5. Identifies and integrates patient population and/or disease state in programmatic goals and objectives.
H. Research
1. Participates in nursing research at a level consistent with educational background and experience.
2. Maintains understanding of current research of others in related clinical areas through review of relevant publications.
3. Participates in grant writing as appropriate.
G. Other
1. Support other areas of the Heart Failure program, including in liaisons with the EP, OR, or PCIS programs, when necessary, as requested by supervisor.
2. Clinic sessions and volume dictated by program leadership.
3. Assists with review and care plan implementation for patient referrals.
4. Facilitates access to and efficiency of outpatient care by direct patient management in clinic setting in conjunction with attending physicians, documentation in outpatient medical record, and integration of inpatient and outpatient services.
5. Helps coordinate outpatient medical therapy with consulting services.

Qualifications

 QUALIFICATIONS:    

Education/Licensure:

  • Master’s in Nursing (MSN) (or) Doctor of Nursing Practice (DNP) (or) PhD Degree in Nursing required.

  • Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner. 

  • Current Adult, Family, or Acute Care Nurse Practitioner Certification by an accredited APRN certifying body required.

  • Valid prescriptive authority from both MA DPH and Federal DEA required.

  • Current APRN License to practice in Massachusetts required.

  • DEA registration required and current APRN License to practice in Massachusetts required.

Experience:

  • 1. Prior clinical Nurse Practitioner experience (minimum 3-5 years in the last 5 years) in the care of cardiac patients, in the ambulatory setting required.  

  • 2. Knowledge of community resources and post/non-acute care providers preferred.  

  • 3. Familiarity with discharge and resource planning is preferred.  

  • 4. Heart Failure Certification (HF-Cert) preferred.

  • 5. Active membership in a related professional organization is preferred.  

  • 6. Expert clinical judgment skills to function in the independent role. 

Skills Abilities and Competencies Required:

  • Excellent interpersonal relationship and communication skills to collaborate with patients' families, staff and community health workers.

  • Ability to work with frequent stress resulting from busy clinical settings and changing organizational climate.

  • Ability to be self-directive and demonstrate initiative in addressing responsibilities of positions.

  • Ability to utilize appropriate problem solving and conflict resolution skills.

WORKING CONDITIONS:

  • Works in a variety of patient care environments where there may be exposure to communicable disease and hazardous materials.

  • Position is on-site/on-campus.
