This alternating -week schedule gives you the
opportunity to practice at the top of your license across the full continuum of
care—one week in the outpatient clinic, the next managing inpatient care as a
hospitalist. You’ll work alongside an experienced interdisciplinary team
committed to delivering safe, compassionate and evidenced -based care while
making a lasting difference in the community we serve.

Position Overview
The Hybrid Nurse Practitioner provides
comprehensive, patient -centered care in both outpatient and inpatient settings.
This provider will assess, diagnose, treat, and manage acute and chronic
conditions while collaborating closely with physicians, nursing teams,
specialists, and ancillary departments to ensure exceptional care delivery and
smooth transitions across the continuum.
This role is ideal for an adaptable and

What You’ll Do
Outpatient Clinic Responsibilities
- Conduct comprehensive patient
evaluations, including medical histories, physical exams, and diagnostic
assessments
- Diagnose, treat and manage acute
and chronic medical conditions within the scope of practice
- Develop and implement
individualized, evidence -based treatment plans
- Prescribe medications and therapies
- Order, perform and interpret
laboratory tests, imaging studies, and other diagnostics
- Provide preventive care, wellness counseling,
immunizations, and health screenings
- Deliver compassionate patient and
family education on disease prevention and management, medication
adherence, and healthy lifestyle choices to support long -term wellness and
improved health outcomes.
- Collaborate with specialists & ancillary
services and coordinate referrals as needed
- Maintain timely, accurate and compliant
EMR documentation
Inpatient Hospitalist Responsibilities
- Manage hospitalized patients
through admissions, daily rounding, and discharges
- Perform history, physical
examinations and develop care plans for admitted patients
- Monitor patient progress and
respond to changes in clinical condition
- Collaborate with physicians,
nursing staff, therapy services, consultants, and case management teams
- Participate in interdisciplinary
rounds and care conferences
- Support safe discharge planning, follow
up care coordination and transitions of care
- Communicate clearly with patients,
families, outpatient providers and care teams
- Complete admission notes, progress
notes, and discharge summaries and required documentation in the EMR
What We’re Looking For
Required Qualifications
- Graduate of an accredited Nurse
Practitioner program (MSN or DNP)
- Active Nurse Practitioner license
in the applicable state
- National certification as an NP
(ANCC, AANP, or equivalent)
- Current DEA registration (or
ability to obtain)
- Current BLS certification; ACLS
preferred or required for hospitalist role
- Ability to obtain hospital privileges
- Prior experience in outpatient
clinic and inpatient/hospitalist settings
- Background in Internal Medicine,
Family Medicine, or Hospital Medicine
- Experience in rural healthcare or
Critical Access Hospital settings preferred
- EMR proficiency

Work Environment & Physical Requirements
This role is performed
in both outpatient clinic and inpatient hospital settings and may require:
- Standing, walking, and sitting for
extended periods
- Assisting or lifting patients when
necessary
- Managing fast -paced and high -acuity
clinical situations and make sound clinical decisions
