Description

Roswell Park Care Network is looking to hire a Nurse Practitioner focused on patients in our new SURVIVORSHIP Program but also caring to the needs of a couple other locations.  The position is primarily based out of Lockport but will travel to our Northtown location in Williamsville as well.

The Nurse Practitioner provides healthcare services to adult patients including health promotion, cancer prevention, with heavy emphasis on survivorship - mental/emotional health, rehab, care for acute and chronic phases of illness, as well as patient and family education and counseling.   Works in collaboration with Social Worker and also our Main Campus team.

ESSENTIAL FUNCTIONS

  • Assesses the physical and psychological status of patients by means of interview, health history, physical examination, and diagnostic studies.
  • Recognizes deviations from normal in physical assessment; works in collaboration with physician in formulating treatment plans for health problems and follow-up.
  • Writes prescriptions for medications and blood products based on laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post discharge care in accordance with written practice protocols.
  • Reviews findings and determines eligibility for entry into clinical trials. 
  • Collaborates with other members of the healthcare team, requests written consultation from physicians and other healthcare professionals as appropriate.
  • Interprets and evaluates findings from diagnostic studies.
  • Relays appropriate actions to facilitate and implement therapeutic plans consistent with the continuing care needs of the patient.
  • Administers therapeutic measures/obtains specimens as appropriate.
  • Evaluates the quality of care provided and recommends changes for improvement in the delivery of care.
  • Initiates appropriate actions to facilitate the implementation of therapeutic plans consistent with the continuing care needs of the patient. 
  • Dictates or writes patient history, admission notes, care plans, progress notes, discharge notes and follow-up letters.
  • Assists in management of family dynamics and coping mechanisms during the acute and chronic phase of the patient’s care.
  • Triages patients appropriately and provides emergency treatment as necessary.
  • Considers the physical, emotional, social, and economic status, as well as cultural and educational backgrounds of individuals, families and communities when providing care.
  • Ensures that departmental and interdepartmental policies, procedures, and standards of care are maintained and assists in interpreting these to patients, visitors, and staff. 
  • Informs supervisors and/or the nurse supervisor of actual/potential problems that require resolution at the administrative level. 
  • Educates patients and families about the disease process and treatment.
  • Provides information to patients and families related to symptom management and the psychosocial response to cancer and its treatment.
  • Identifies the learning needs of the nursing and medical staff and students and implements educational programs as appropriate.
  • Provides formal and informal education to the nursing staff.
  • Identifies, analyzes, and resolves patient care problems, resulting in system improvements or improvements in patient care.
  • Participates in continuous quality improvement/organization operating in system improvements or improvements in patient care. 
  • Participates in continuous quality improvement/organization operating procedure activities.
  • Demonstrates a commitment to continuously improving the quality of care provided to patients.

EDUCATION & EXPERIENCE

  • Master’s degree in Nursing from an accredited school, college, or university or one recognized by Roswell Park Community Practices. 
  • Experience with survivorship care, including assessing long-term physical and emotional needs of cancer survivors.
  • Knowledge of survivorship guidelines, health maintenance needs, and interventions that promote quality of life post-treatment.
  • Ability to create and implement individualized survivorship care plans, addressing follow-up screenings, lifestyle adjustments, and monitoring for recurrence or late effects of treatment.
  • Familiarity with coordinating a survivorship program, potentially including setting up processes, educating staff, and monitoring program effectiveness.
  • Skills in collecting and analyzing data on survivorship outcomes and patient satisfaction to improve program services
  • Experience in an oncology and hematology setting
  • Familiarity with chemotherapy, immunotherapy, targeted therapy, and supportive care

CERTIFICATIONS/ LICENSES

  • License to practice as a registered Nurse in the State of New York.
  • Current Nurse Practitioner Certification in New York.
  • Certification or eligibility for certification as a Nurse Practitioner by the American Nurses Credentialing Center or other national certifying body recognized by the Health Care Financing Administration (HCFA).
  • As a condition of continued employment, incumbents must obtain certification as a Nurse Practitioner by the American Nurses Credentialing Center or other national certifying body recognized by the HCFA within nine (9) months of their appointment to this title as required by Medicare. 
  • Certification Basic Life Support (BLS).
