Job Summary

We are seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join Duke Cardiology of Raleigh in an outpatient Advanced Heart Failure (AHF) role. The Advanced Heart Failure (AHF) Advanced Practice Provider (APP) is a Nurse Practitioner (NP) or Physician Assistant (PA) who provides direct patient care services and performs medical acts as an NP or PA, under the supervision of the supervisory physician, to patients with AHF in an outpatient setting.

The APP collaborates with patients and the multidisciplinary team to formulate and carry out a plan of care for patients being evaluated and treated for advanced HF. The APP collaborates with the Advanced Heart Failure Medicine team at Duke Raleigh Hospital and Duke University Health System (DUHS).  The advanced HF APP performs physical exams, collects data and documents, conducts diagnostic and therapeutic procedures, analyzes clinical data, orders and schedules laboratory studies, prescribes appropriate interventions and medications, and provides direct patient care services.

The AHF APP contributes to excellence in patient care, education and teaching and provides leadership to the organization.

Job Duties & Responsibilities

• Evaluate established adult cardiovascular/heart failure patients and patients requiring early post-discharge follow-up


• Assess and diagnose clinical conditions, develop treatment plans, establish priorities and outcomes, and evaluate the effectiveness and cost efficiency of care.


• Perform comprehensive histories, physical examinations, and medical decision-making; develop and implement plans of care, including medication reconciliation and titration; and document patient encounters, findings, diagnostic results, treatment plans, responses, and communications with the supervising physician.


• Order, review, interpret, and communicate laboratory and diagnostic study results.


• Facilitate and coordinate testing and procedures such as echocardiograms, stress testing, cardiac MRI, cardiac catheterization, laboratory studies, and other indicated evaluations in collaboration with the supervising physician.


• Educate patients and families regarding diagnoses, medications, treatment plans, procedures, preparation, risks, benefits, and expected outcomes.


• Coordinate care with referring and consulting providers, obtain outside records and test results, and ensure timely communication among the healthcare team.


• Manage MAESTRO in-basket messages and complete required clinical and administrative documentation.


• Perform procedures and utilize prescriptive authority within approved scope of practice, credentialing, practice agreements, and applicable North Carolina regulations; consult and collaborate with the supervising physician as appropriate


• Provide patient/family and staff education; serve as a clinical resource and role model; assist with orientation and training of new employees, students, and trainees; incorporate evidence-based practice and current research into care; and participate in quality improvement, research, professional development, and required supervisory/performance evaluation activities.

Job Eligibility Requirements

Education           

• Nurse Practitioners:  Work requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.


• Physician Assistants:  Work requires completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.

Certification 

• National certification as an NP or NCCPA certification as a PA required.


• Current DEA required.


• ACLS required.


• BLS required.

License                

• Nurse Practitioners:  Must have current or compact RN licensure & NP approval to practice in the state of North Carolina. BLS required. National certification as a NP required.


• Physician Assistants:  Must be licensed in the state of NC, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants. BLS required.


• DEA licensure with full prescriptive authority required.

Experience          

• Strongly prefer a candidate with at least two years of experience as an NP/PA in cardiology, including experience caring for heart failure patients.

Job Hours

Primary hours: This is a full-time salaried position, working a 40-hour work week (5 Shifts/week).  This would include a minimum of 8 patient-facing sessions of at least 3.5 hours of templated clinic time.  There would be dedicated administrative time (8 hours) per week.  Remote patient encounters / telehealth visits are permitted.  

Variations in hours and days of the week covered may be changed by Heart Center Leadership in consultation with the clinical service line leadership based on service needs
