Position Details:


The Hospital of Central Connecticut is seeking a Nurse Practitioner or Physician Assistant to join their Palliative Care team! The Advanced Practitioner (APP) will work in collaboration with the attending physician and members of the interdisciplinary care team to deliver expert palliative care services to patients and their families living with chronic serious or acute life threatening illness, or those nearing end of life. The APP is an expert in pain and symptom management, seeking to optimize patient function and quality of life within the limits of the disease process, while considering factors such as fear, communication barriers, economic issues and cultural traditions.


 


Role: Physician Assistant or Nurse Practitioner


Location: 100 Grand Street, New Britain, CT


Setting: Inpatient, Palliative Care


Schedule:  Full-time, Monday-Friday.


Call schedule: No call

Physician Assistants/Nurse Practitioners Role Scope:


Under established guidelines and in collaboration and/or supervision with the practice physician(s), Physician Assistants/Nurse Practitioners perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.


In consultation with the physician(s) and care team members, Physician Assistants/Nurse Practitioners develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.


Physician Assistants/Nurse Practitioners provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Qualifications

Requirements:



  • Licensure: Unrestricted State of Connecticut Medical License (or eligible)

  • Training: Physician Assistant or Nurse Practitioner

  • Preferred Experience:  1+ years’ experience preferred


    • National Board Certification in Palliative Care
