VNA Hospice NWI is seeking:

Nurse Practitioner-Palliative Care/Chronic Care Management

PRN - one day per week, day shift

Coverage Area includes: Porter, LaPorte, Starke, Jasper, Newton and Lake County. Mileage paid at 67 cents per mile.

SUMMARY:

PRN nurse practitioner needed in growing palliative care and chronic care management program. This position visits patients in their homes, facilities and our 13-bed in-patient center. Join a fast-paced team with competitive pay.  Additionally, you will build meaningful relationships with the residents and their families.

Our Nurse Practitioners collaborate with the, Medical Director and Palliative Clinical Director to ensure that quality Palliative and Chronic Care is being provided to our patients.

Requirements

QUALIFIED CANDIDATE WILL POSSESS:

• Current, unrestricted NP license in the State of Indiana
• A clinical background in adult, family or geriatric specialties.
• DEA or ability to obtain
• Prescriptive authority or ability to obtain
• Minimum 1 year NP experience
• Ability to work autonomously
• Reliable transportation and auto insurance
• Ability to travel throughout our service area
• Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook

Preferred Qualifications:

• Long term care and/or nursing home based experience
• Experience working with the older adult and/or geriatric population
• Palliative care experience
• Chronic care management experience
• Home care experience
• Hospital based experience
• Oncology experience
• Critical care experience
• Familiarity with remote monitoring/telehealth

Medical specialties:

• Palliative Medicine, Geriatric , Chronic Care Management

Standard shift:

• Day shift 8:00 a.m. - 4:30 p.m.

Experience:

• Relevant: 1 year (Preferred)
• Nurse Practitioner: 1 year (Required)

License/Certification:

• Certified Nurse Practitioner (Required)
