Job Description - Nurse Practitioner Whiting NJ

Nurse Practitioner Whiting NJ
We are looking for a Nurse Practitioner to join our medical group full time in Whiting and Freehold NJ. The incoming Nurse Practitioner will be required to see and treat a wide variety of health care issues in patients, providing comprehensive medical care and management primarily for geriatric, sub-acute patients, including admission intake, diagnosis, treatment, and preventative care interventions tailored to the unique needs of older adults. May be asked to provide care to Long Term Care/Skilled, sub-acute, Assisted Living and medical office patients as needed. The Nurse Practitioner will be managing complex medical conditions commonly associated with aging, while prioritizing patient safety, comfort, and quality of life. The calls are very minimal and would be primarily from independent living and a very small, memory care household. We have Physician coverage during off-hours for the other levels of care. On occasion, a call for sub-acute, skilled nursing or assisted living may come through if the Physicians is not able to respond in 15 minutes.
Duties:

Provide direct care to patients
Handle admissions and quarterly reviews
Evaluates and treats common acute and chronic conditions -Conduct thorough geriatric assessments to eval physical, cognitive, functional and other aspects of health
Orders and reviews diagnostic tests, X-rays, labs, and bloodwork
Consistently and accurately documents all calls, treatments, medications, etc. in patient chart
Instructs and counsels patients regarding diagnosis and follow up steps
Responds to all patient questions
Participate in chart reviews as needed
Collaboration with physicians, advanced practice providers, and support staff for comprehensive care
Coordinate with specialists, allied health professionals, and community resources to ensure seamless continuity of care and support services
Attempt to minimize rehospitalizations and minimize length of stay with positive outcomes

Requirements:

Nurse Practitioner License in NJ
2+ years of relevant experience preferred with strong clinical skills and experience providing primary care to adult patients
Familiarity with geriatric medicine and a passion for working with older adults
Ability to provide coverage in both Freehold and Whiting

Schedule:

4 days per week (32 hours per week- 3 days per week in Freehold and 1 day per week in Whiting)

Compensation (range):

$135k-$150k plus additional pay for on-call coverage
