Job Description Summary

‎ 

We are seeking a dedicated and clinically astute Nurse Practitioner to join our geriatric clinical team. In this role, you will serve as the primary clinical presence within assigned Skilled Nursing Facilities (SNFs), bridging the gap between acute hospital discharge and long-term recovery.

You will manage a diverse patient population ranging from short-term rehabilitation residents to long-term care patients, ensuring high-quality, person-centered medical management.

Key Responsibilities

  • Clinical Excellence & Rounding
    • Comprehensive Assessments: Conduct regular clinical rounding, performing detailed physical exams and assessments to formulate accurate diagnoses and treatment plans.
    • Complex Geriatric Care: Manage intricate geriatric syndromes, including cognitive impairment, mobility issues, and frailty.
    • Acute & Chronic Management: Oversee the medical needs of patients with multiple chronic comorbidities and those requiring acute post-hospitalization rehabilitation.
    • Medication Management: Review and optimize medication regimens with a specific focus on reducing polypharmacy and ensuring pharmacological safety in the elderly.
  • Collaboration & Communication
    • Physician Partnership: Collaborate closely with the attending physician to co-manage complex cases in both SNF and long-term care settings.
    • Interdisciplinary Leadership: Actively participate in Interdisciplinary Team (IDT) meetings alongside nursing staff, therapists, social workers, and families to create and implement cohesive care plans.
    • Goals of Care: Lead sensitive and critical conversations with residents and their families regarding advance care planning, palliative goals, and quality-of-life transitions.
  • Administrative & On-Call Requirements
    • Complete accurate patient documentation in a timely manner
    • Clinical Integration: Participate in weekly clinical conference calls to discuss patient outcomes and practice standards.
    • Professional Development: Attend in-person practice meetings approximately four (4) times per year.
    • On-Call Coverage: Shared on-call responsibilities totaling approximately 12 shifts per year.

Qualifications

  • Licensure: Active, unrestricted APRN license in the state of Texas.
  • Certification: Board certification. Geriatric certification is a plus.
  • Experience: Prior experience in geriatrics, internal medicine, or post-acute care is highly preferred.
  • Skills: Strong clinical intuition, autonomy in decision-making, and exceptional "bedside manner" when handling end-of-life or goals-of-care discussions.
