Integral Health Partners is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team. We are looking for full-time and part-time candidates to work at least 3-5 days per week. This position includes working in skilled nursing facilities, rest homes and ALF’s in the greater Worcester area, as well as Framingham/Natick.

Nurse Practitioner/Physician Assistant Responsibilities:

• Conducts comprehensive health assessments
• Formulates diagnosis and treatment plans for both chronic and acute medical conditions
• Provides care in accordance with standards of practice and in compliance with regulatory agencies
• Obtains comprehensive health histories
• Completes in depth physical exams
• Analyzes data and diagnostics
• Partners with entire care team to formulate a plan of care documents in a timely manner
• Provides patient and caregiver education
• Complying with HIPAA Laws

Nurse Practitioner/Physician Assistant Requirements:

• 1-3 years of NP and/or PA experience preferred
• Completion of NP or PA accredited program
• Board certification
• Current unrestricted NP or PA state license
• Current DEA license
• BLS certification
• Driver’s license
• Willingness to conduct house calls as needed
• Access to reliable transportation to conduct patient visits
• Passion for delivering care to geriatric patient’s with complex medical needs
• Excellent communication skills
• Evidence of immunization including TB, MMR, Tdap, influenza and COVID in compliance with federal and state regulation

Compensation & Benefits:

Salary: 100k-110k depending on hours and experience.
