Job Description - Nurse Supervisor (NFP)

Description

PRIMARY FUNCTION/PURPOSE 

The function of the Nurse-Family Partnership Nurse Supervisor is to provide leadership and guidance to all Nurse-Family Partnership (NFP) activities and Nurses. The NFP Nurse Supervisor uses principles of supervision, reflective practice, and staff development to provide primary support and appropriate oversight to the Nurse Home Visitors and Administrative Assistant (AA) to build and model a strengths-based, client-centered culture. The NFP Nurse Supervisor works within the local community and agency to ensure that the NFP program is implemented in accordance with established guidelines and contract requirements. 


 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Implement, monitor and maintain Nurse-Family Partnership activities and procedures in accordance with the national model requirements and guidelines set forth by The Children's Shelter. 
  • Oversee the day-to-day NFP program and partnerships to include budgeting, training, staffing, recruiting, and community-based activities. 
  • Understands and continually models the philosophy of the NFP model. 
  • Ensure that all staff, including self, attend all required NFP training. 
  • Work with AA to assign client caseloads and monitor home visitor's ability to build and manage caseload. 
  • Provide reflective supervision and evaluation of NFP home visitors and AA. 
  • Maintain client files, charting activities, progress, contact time and other vital information as needed or required by the national model and The Children's Shelter requirements. 
  • Maintain accurate and timely client records in the agency and NFP database. 
  • Maintain data and reports as required by agency and Chief Program Officer. 
  • Participate in recruitment activities in partnership with other agency programs. 
  • Participate and assist with planning of collaborative department events such as Quarterly Baby Showers and Trunk or Treat. 
  • Develop and maintain community referral sources for the NFP program. 
  • Develop and implement external and internal systems to assure timely receipt and disposition of referrals to and from the NFP. 
  • Develop and implement regular joint home visits with nurses in the field. 
  • Plan and expend budget to ensure compliance with fidelity to NFP model and contract agreements. 
  • Understand and actively participate in monitoring NFP contract with the agency. 
  • Coordinate and actively participate in all NFP community advisory board meetings. 
  • Actively participates in agency Strategic Planning initiatives. 
  • Demonstrates strong commitment to meeting the needs of our customers (children, community members, co-workers, donors, managers and volunteers), in a friendly, outgoing and professional manner. 
  • Responds to customers in a timely and thorough manner; does what is necessary to ensure customer satisfaction; prioritizes customer’s needs and follows up to evaluate customer’s satisfaction. 
  • Coaches to create structured, focused interaction with learners and uses appropriate strategies, tools, and techniques to promote desirable and sustainable change for the benefit of the learner, making a positive impact on the organization. 
  • Performs other duties as assigned by Senior Director of Family Strengthening. 

GENERAL EXPECTATIONS 

  • Submit assigned program reports to the Sr. Director of Family Strengthening. 
  • Submit required reports to DFPS/PEI and/or NSO to include monthly, quarterly and annual reports. 
  • Collaborate with internal and external programs. 
  • Ensure program participation at peer records review. 
  • Report on program data and outcomes at quarterly PQI meetings, and other meetings as requested. 
  • Acquire 25 training hours annually in accordance with agency policy. 
  • Be consistent and reliable with punctuality and attendance. 
  • Care for the property of The Children's Shelter, reporting necessary repairs/needs. 
  • Be proactive with local, state and national issues that may affect sustainability of program. 
  • Build confidence and skill of home visitors. 
  • Communicate and cooperate with agency partners, co-workers and staff in a respectful and effective manner. 
  • Read and interpret NSO database reports; share appropriately with team members. 
  • Executes responsibilities with cultural sensitivity to groups and individuals. 
  • Attend staff meetings and assist the VP of Programs in staff meetings and trainings, when requested. 
  • Responsible for own personal safety, your co-workers, and others that might enter your work area. 
  • Contributes to building a child welfare system that is safe, secure, and stable; family centered; connected to the community; which relies on evidence-based practice necessary to support contract services. 
  • Adheres to all policies and procedures of the agency and its accrediting and standard setting bodies, such as Council on Accreditation (COA) and state licensing. 
  • Ensures all processes and tasks completed meet local, state, and/or federal compliance requirements. 
  • Maintains client confidentiality per The Children’s Shelter policies and all applicable codes of ethics. 
  • Meets assigned deadlines and follows through on assignments. 
  • Learns, understands, supports, and demonstrates The Children’s Shelter mission, vision, and values. 
  • Accepts feedback, coaching, and supervision. 
  • Demonstrates the Trauma Informed Care principles and utilizes with peers, clients, and external stakeholders. 
  • Understands and demonstrates the Praesidium best practices to ensure the safety of clients. 
  • Promotes teamwork within team, program, and agency. 
  • Promotes and demonstrates appropriate respect for cultural diversity among coworkers and clients. 
  • Attends and successfully completes mandatory trainings as defined in positions training matrix by the assigned deadline. Submits certifications of trainings to Training Department. 
  • Uses outside professional collaboration and intra-agency teamwork appropriate to the agency. 
  • Avoid all conflicts of interest, real or perceived. 
  • Attends work in accordance with agency leave policy. 

Requirements

REQUIREMENTS 

  • A minimum of a Bachelor’s degree in nursing with two years of paid nursing experience in maternal/child health.  
  • Minimum of 2-4 years of nurse supervisor experience in nursing and/or 1-2 years of experience as an NFP Nurse Home Visitor. 
  • Current RN license in good standing and CPR certification required. 
  • Experience with supervision and program development. 
  • Experience in organizing and implementing meetings and activities. 
  • Proficient in Microsoft office, including MS Word, MS Excel, and MS PowerPoint. 
  • Sufficient maturity and judgment to function in crises. 
  • Ability to seek supervision and use it constructively. 
  • Ability to have a joyful approach to children, adolescents, and families of diverse cultures. 
  • Must maintain a Texas driver's license and provide proof of automotive liability insurance. 

KNOWLEDGE, SKILLS, AND ABILITIES: 

• Strong computer skills, specifically in the use of Microsoft applications and client databases. 

• Ability to enter client data into the DMCN and PEIRS.  

• Ability to plan, organize and coordinate work assignments independently. 

• Ability to communicate effectively, clearly, and concisely through oral and written means. 

• Ability to establish and maintain effective working relationships with others. 

• Ability to understand and apply applicable rules, regulations, policies, and procedures. 

• Ability to work independently, with minimal direct supervision and demonstrate self-motivation. 

• Ability to develop and maintain written documentation. 

• Knowledge of the target population. 

• Knowledge of and sensitivity to cultural and linguistic competence. 

• Ability to prioritize tasks and meet deadlines. 

• Skilled in organizing work in an environment with multiple challenges and priorities. 

• Ability to demonstrate exceptional customer service skills. 

• Ability to follow agency security and safety guidelines. 

• Ability to apply continuous quality improvement tools and techniques to monitor outcomes and improve efficiencies.  

• Background check, drug testing and TB screening required post-offer prior to employment. 

DESCRIPTION OF THE WORKING ENVIRONMENT 

This is a full-time position, scheduled for 40 hours a week with the need for a flexible schedule.Work environments of the Nurse-Family Partnership- Nurse Supervisor consists primarily of office and community activities, including community outreach. Traveling to designated locations will be required regularly.
