Job Description - NUTURING FAMILIES - FACILITATOR/CASE MANAGER

Description

DESCRIPTION OF POSITION:

The primary responsibility of this position is to facilitate Nurturing Families parenting classes both in person and virtually in the office and community and to provide targeted case management services.

  

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  1. Facilitate Nurturing Parent classes in person and online.
  2. Prepare classroom location and all classroom materials in preparation for each class. Have class materials readily available in appropriate quantities at all times. 
  3. Assist with providing Child Enrichment classes.
  4. Assist the Program Supervisor in meeting all contract requirements.
  5. Maintain class logs and tracking system to document classes completed by clients.
  6. Complete targeting case management services and all required require assessments. 
  7. Responsible for researching and coordinating services to meet the needs of participants as directed by Program Director or Program Supervisor (i.e. housing, employment, education, mental health)
  8. Assist parents with community referrals for all related services and assisting parents with accessing referrals, providing transportation as needed. 
  9. Attend CFT meetings organized by DCFS
  10. Assist the Coordinator with collection of data
  11. Represent Walden’s Nurturing Parent Program in the community and attend required community, county and First 5 meetings.
  12. Assist in community outreach events, a minimum of one event per quarter.
  13. Research potential training tools and program enhancements. 
  14. At all times projects a favorable image of Walden Family Services.
  15. Perform other duties as assigned or requested.

The employee is frequently required to reach with hands and arms.

The employee is occasionally required to climb, stoop, kneel and crouch or crawl.

Employee performs occasional computer work.

Employee must occasionally lift and/or move up to 15 pounds.

Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

Requirements

QUALIFICATIONS REQUIREMENTS:

  1. Bachelor’s degree in social work or related field. 
  2. The position requires that the individual have excellent public speaking and communication skills. 
  3. Previous experience delivering clinically based training.
  4. Previous experience working with children and families in a social services setting preferred.
  5. Bilingual ( Spanish / English ) preferred. 
  6. Available evenings and some Saturdays.  
  7. Good organizational skills. Ability to write training curriculum.  
  8. Ability to communicate with individuals from diverse multicultural and socioeconomic backgrounds.  
  9. Must possess valid California Driver’s License and have a good driving record.
  10. Fingerprint and Child Abuse Index clearances from the Department of Justice.
  11. Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility.

EMPLOYEE STATUS: Full Time – Non-Exempt

SALARY RANGE: $22.00 - $24.00

PRIMARY SUPERVISOR: Nurturing Families Supervisor or Program Director.

COVID VACCINATION REQUIRED.
