Job Title: Field RN – Home Health Admissions

Job Responsibilities:

• Complete Start of Care OASIS admissions
  
• Create and implement care plans
  
• Document accurately and efficiently
  
• Collaborate with care team for seamless transitions
  

Requirements:

• Active Minnesota RN license
  
• Prior OASIS and SOC experience
  
• Ability to work independently and communicate clearly
