Job Description - Occupational Therapist

Purpose:

OT will work with Class Members to help them develop, maintain, or restore physical and mental functioning through engagement in meaningful and productive occupations, including ADLs, IADLs, rest and sleep, education, work, play, leisure, health management, and social participation.

Essential Responsibilities, include but are not limited to:

  • Evaluate a Class Member’s performance of ADLs, IADLs, and functional mobility
  • Assess personal, environmental, medical, and social variables that influence a Class Member’s ability to do what they want and need to be able to do consistently, safely, and with adequate skills 
  • Assess a Class Member’s ability and capacity to transition, and make recommendations for supportive services and the most appropriate community setting 
  • Develop and implement a treatment plan based on needs and Class Member’s goals, to support successful community transition 
  • Attend meetings and consult with team regarding Class Member’s progress and share recommendations 
  • Collaborate with interdisciplinary professionals (i.e. Physicians, Therapists, Psychiatrists, Nurses) for comprehensive care to support community living 
  • Comply with assessment, treatment, and documentation guidelines from Envision Unlimited, DHS, and the IL OT Practice Act
  • Perform other related duties or projects as assigned

Requirements

The requirements listed below represent the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

EDUCATION, SKILLS, and/or EXPERIENCE:

  • Master’s degree in Occupational Therapy w/ license to practice in Illinois. 
  • Prefer someone with experience but will consider internships for new graduates.
  • Must have valid drivers license
  • Must be able to pass background check.

Compensation & Benefits:

  • Base Salary: $80,000 per year 
  • Paid Time Off: 11 Vacation days, 12 Holidays days, 3 sick days, 3 personal days
