We are currently seeking a full-time Occupational Therapist (OT) to join our team in North Port, Englewood, and Charlotte County, FL.
About the Role
As an Occupational Therapist at Haven, you will partner with patients throughout their plan of care. Your focus will be on restoring motion and function, ensuring safety in the home, and maximizing quality of life. You will apply your expertise in promotion, prevention, diagnosis, treatment, intervention, and rehabilitation to support healthy recovery.
Under the direction of the physician’s Plan of Treatment and in collaboration with the patient and caregiver, you will help each patient achieve their personal best and maintain independence.

Key Responsibilities
Adhere to established policies, procedures, and professional standards.
Improve or minimize residual physical disabilities of patients.
Return patients to their optimum, productive level within their capabilities.
Collaborate with healthcare personnel in patient care planning.
Conduct initial patient evaluations and take histories.
Perform skilled OT procedures as ordered by the physician.
Consult with physicians regarding changes in treatment.
Educate patients and families on home programs and activities of daily living.
Attend patient care conferences as scheduled.
Prepare and maintain clinical and progress notes.
Develop and revise the plan of care as needed.
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with agency timelines.
Qualifications
Current Occupational Therapy License in the state of Florida
Two (2) years of home health experience preferred
Acceptance of the philosophy and goals of Haven Home Health Care
Ability to exercise initiative and independent judgment
Strong clinical expertise and ongoing commitment to learning
Ability to manage patients with diverse personalities and needs
Excellent interpersonal and communication skills
Professional behavior and strong work ethic
