Job Description - Occupational Therapist

About Oakwood Creative Care


Oakwood Creative Care is a nonprofit organization dedicated to transforming how communities support healthy aging, brain health, and people living with dementia. Through day clubs, family support hubs, and healthy living and resilience clinics, we help individuals remain connected, purposeful, and engaged throughout the aging journey.


Position Summary:


The Occupational Therapist is an integral part of the OCC interdisciplinary team. The OT is a dementia expert that evaluates the person with dementia to determine strengths and areas needing intervention through creative adaptation to retain existing function and independence in activities of daily living for as long as possible, prolong institutionalization, enhance quality of life, and support family care partners.


Key Responsibilities:



  • Exemplify OCC’s core values while ensuring continuous innovation and development in methodologies and interventions to delay functional decline and reduce the challenging behaviors that trigger institutionalization by offering hands-on education and support to understand dementia, reduce challenging behaviors, promote function, communicate effectively, make the home safer, and optimize independence.

  • Provide training and education to family care partners on how to persevere existing capabilities by simplifying tasks, improving communication, managing difficult behaviors, increasing activity engagement, and conducting home safety assessments to provide recommendations to achieve success.

  • Identify and meet client’s specific goals/needs, offer treatments to help improve activities of daily living/enhance quality of life, develop care plans, complete progress notes, evaluate and monitor treatment plans.

  • Facilitate community education classes on topics such as healthy aging, brain health, resilience, and dementia care. Participate in workshops, outreach events, Qualifications


Requirements



  • Master’s or Doctorate in Occupational Therapy

  • Current Arizona Occupational Therapy license

  • Experience working with older adults, dementia, or neurocognitive conditions preferred

  • Strong understanding of person-centered care and functional independence

  • Excellent collaboration and communication skills

  • Passion for innovative, community-based models of aging services
