Position Details

• Full-Time with flexible Monday–Friday scheduling


• Provide skilled in-home occupational therapy services


• Develop individualized treatment plans to improve daily living skills and functional independence


• Service Areas: Charlotte County and surrounding areas, including Punta Gorda, Port Charlotte, Englewood, North Port, and nearby communities

Responsibilities

• Deliver skilled occupational therapy services in patients’ homes


• Develop, implement, and update individualized treatment plans


• Evaluate and reassess patient progress, adjusting plans of care as needed


• Collaborate with physicians, nurses, and interdisciplinary team members


• Educate patients and families on techniques to improve safety, function, and independence

Qualifications

• Current Florida Occupational Therapist license (or ability to obtain prior to start)


• Home health experience preferred; willing to train the right candidate


• Strong clinical judgment, communication, and organizational skills


• Compassionate, patient-centered approach to care
