Occupational Therapist (LTC/SNF) – Travel Position

Pay: $2,085/week

Job Description:

We are seeking a compassionate Occupational Therapist to join a skilled nursing and long-term care facility in Medford, OR, on a travel contract. The Occupational Therapist will evaluate residents, develop individualized treatment plans, and provide therapy services focused on helping patients regain independence in daily living and functional activities. The ideal candidate is patient-centered, adaptable, and experienced in geriatric rehabilitation.

Key Responsibilities:

• Conduct patient evaluations to assess functional abilities, cognitive skills, and therapy needs.


• Develop and implement personalized treatment plans in collaboration with physicians and care teams.


• Provide therapeutic interventions focused on self-care, mobility, fine motor skills, and adaptive techniques.


• Educate residents, families, and caregivers on therapy goals, safety practices, and assistive devices.


• Document evaluations, treatment sessions, and progress notes accurately and timely.


• Collaborate with interdisciplinary staff to ensure coordinated, patient-centered care.


• Support resident safety, independence, and quality of life through evidence-based therapy practices.

Qualifications:

• Master’s or Doctorate degree in Occupational Therapy from an accredited program.


• Current and valid Occupational Therapist license in Oregon (or compact license).


• Minimum of 1 year of recent OT experience, preferably in long-term care or skilled nursing.


• Current certification in Basic Life Support.


• COVID-19 vaccine and recent tuberculosis test.


• Strong clinical, communication, and organizational skills.


• Compassionate, patient-centered approach with experience supporting geriatric populations.
