We are building Collier County’s first rapid -response home care team.

This position is for caregivers who are willing to:

• Accept short -notice assignments

• Cover hospital discharges

• Prevent rehospitalization

• Provide immediate stabilization care

Higher pay available for:

â Nights

â Weekends

â Emergency placements
