Job Description:

As an On-Call Hospice RN, you will be crucial in providing high-quality end-of-life care to our patients and supporting their families during challenging times. You will be responsible for responding to after-hours calls and providing skilled nursing care, emotional support, and symptom management as needed. This position requires flexibility and a deep commitment to delivering exceptional care to our patients in their homes, hospice facilities, or other settings as required responsively.

Key Responsibilities:

1. Respond promptly to after-hours calls from patients, families, and caregivers.


2. Conduct patient assessments to evaluate pain, symptom management, and overall comfort.


3. Administer medications, perform wound care, and assist with medical procedures as necessary.


4. Provide emotional support and education to patients and their families regarding end-of-life care.


5. Collaborate closely with the interdisciplinary hospice team, including physicians, social workers, chaplains, and volunteers.


6. Ensures that arrangements for equipment and other necessary items and services are available.


7. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.


8. Maintain accurate and thorough patient records in compliance with hospice regulations, the organization's policy, and best practices.
   


9. Ensure the delivery of compassionate and culturally sensitive care tailored to each patient's unique needs.
   


10. Participate in on-call rotation schedules, including weekends and holidays, to ensure 24/7 availability for patients.
    

Qualifications:

1. Current Registered Nurse (RN) license in the State of Georgia and current CPR certification.


2. Minimum of three (3) years of nursing experience, with previous hospice or palliative care experience preferred.


3. Strong clinical assessment and critical thinking skills.


4. Excellent communication and interpersonal abilities.


5. Compassionate and empathetic demeanor.


6. Ability to work independently and as part of a collaborative team.


7. Reliable transportation and a valid driver's license.


8. Willingness to work on-call shifts, including nights, weekends, and holidays.
