The On-Call Hospice LVN provides compassionate and skilled nursing care to patients receiving hospice services during non-business hours. The LVN responds to patient needs, provides symptom management, and offers support to patients and families during end-of-life care.
Responsibilities:
Provide direct patient care to hospice patients in their homes or hospice facilities during non-business hours.
Conduct comprehensive assessments of patient needs, including physical, emotional, and spiritual aspects.
Administer medications, treatments, and interventions as prescribed by the hospice interdisciplinary team.
Monitor patient vital signs, symptoms, and overall condition, reporting any changes or concerns promptly.
Communicate effectively with the hospice interdisciplinary team, including physicians, registered nurses, social workers, and chaplains.
Offer emotional support and comfort to patients and families, fostering a caring and compassionate environment.
Document all patient care activities accurately and thoroughly in the electronic medical record.
Provide education and guidance to patients and families regarding symptom management, medication administration, and end-of-life care.
Collaborate with other healthcare providers and community resources to ensure continuity of care for hospice patients.
Participate in ongoing education and training to maintain licensure and enhance clinical skills.
Requirements
Hospice LVN Requirements:
Graduation from an accredited Licensed Vocational Nurse program.
Current LVN license in the state of practice.
Minimum of two years of nursing experience, preferably in hospice or palliative care.
Strong assessment, communication, and interpersonal skills.
Ability to work independently and make sound clinical judgments.
Compassionate and empathetic approach to patient care.
Willingness to work non-business hour shifts.
Valid driver's license, reliable transportation, and auto insurance.
