Oncology Care Navigator

Rate Range: $32-$38 per hour

Coordinates the care and services of patients across the continuum of their cancer journey. Assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes.

Requirements:

Master's degree in Social Work from an accredited school of Social Work. Minimum of one (1) year experience in oncology preferred. Strong clinical, organizational, and interpersonal skills. Ability to read, write, and communicate in English

New York State Licensure to practice Social Work in the State of New York.
