• Call Schedule: Call will be at least one week a month to include the weekend. Rotating call coverage among the providers in the facility.

• Call Type: Phone

• Call Response Time: On -call providers must be available within 15 minutes by phone and onsite within 60 minutes.

• Phone Consults: Yes

• Rounding included: Yes

• % of Practice: 95% Direct Patient Care, 5% Administrative Duties

• Procedures: Comprehensive clinical ophthalmology procedures, including the full range of oculoplastic and glaucoma services.

• EMR: CPRS

• Hospital Privileges required: Yes

• Temporary Privileges available: Yes

• Travel, lodging, and malpractice insurance covered

Requirements:

• Active unrestricted license in any State or U.S. territory

• Board Certified/Board Eligible by the American Board of Ophthalmology (ABO)

• BLS

• ACLS

• DEA License

• NPI

• NPDB Report

Please note: Physicians who have ever had a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
