Position Overview:

We are seeking a skilled and motivated Outpatient Manager (Physical Therapist) to lead our outpatient clinic in Boca Raton, FL. This leadership role is ideal for an experienced PT who thrives in a collaborative environment and is passionate about patient-centered, multidisciplinary care.

Key Responsibilities:

• Provide clinical leadership and day-to-day management of the outpatient therapy team.
• Ensure efficient utilization of visits and maintain compliance with regulatory standards.
• Perform comprehensive physical therapy evaluations and re-evaluations.
• Design and implement individualized treatment plans that align with patient goals.
• Educate patients and caregivers on therapeutic techniques and home exercise programs.
• Collaborate closely with occupational and speech therapists to support coordinated care delivery.
• Track and document patient outcomes, modifying care plans as needed.
• Support seamless transitions of care, including discharge planning and referrals for in-home services.
• Maintain accurate, timely documentation in accordance with state and federal guidelines.

Requirements

Qualifications:

• Active license as a Physical Therapist (PT) is required.
• Minimum of 2 years of clinical experience in an outpatient setting preferred.
• Experience in home health or hospital environments is a plus.
• Strong clinical knowledge in geriatrics and neurological conditions such as Parkinson’s disease.
• Excellent leadership, communication, and organizational skills.
• Demonstrated ability to manage schedules and documentation efficiently.
• Experience in interdisciplinary care planning and team coordination is strongly preferred.
