Description

• Interprets the appropriateness of a medication order before preparing or permitting the distribution of the first dose
• Recognizes non-formulary drugs, interacting drugs or therapy intervention targeted drugs, and offers information on alternate choices to physicians, consistent with hospital formulary system.
• Ensures adequate supply of medications and supplies are available beyond the immediate shift.
• Verify all dosage calculations based on patient-specific parameters provided on the patient care plan.
• Consistently completes daily audits of narcotic sheets or ADM records and maintains perpetual inventory of narcotics per policy and procedure

• Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD required
