Outreach and Enrollment Coordinator - Fredericksburg - Bilingual

Description

The Outreach and Enrollment Coordinator will work to provide outreach opportunities for community residents who lack access to healthcare and health insurance, provide enrollment and application follow-up assistance and build/strengthen enrollment collaborations with local partner organizations. This position will be responsible for assisting patients who present with Social Determinants of Health (SDOH) by connecting the patient, family, and care givers with appropriate community resources.  

 Duties and Responsibilities

 a. Provide enrollment assistance (including but not limited to completing coverage applications, gathering required documentation and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace, Medicaid, Children’s Health Insurance Program (CHIP) and Medicare.

b. Provide patient education on health coverage, engage in follow-up conversations and offer insurance renewal assistance for individuals.

c. Distribute outreach materials to patients, community members, partner organizations and businesses to build awareness of outreach opportunities within Frontera.

d. Attend community events to promote outreach and enrollment services.

e. Collaborate with Frontera staff and various local organizations to build awareness of services and build referral linkages.

f. Participate in and successfully complete required training programs and educational opportunities related to the Affordable Care Act Marketplace, Medicaid, CHIP, and Medicare enrollment standards and competencies

g. Accurately work assigned reports and provide accurate data as needed.

h. Assist in the development and implementation of organizational outreach and enrollment initiatives.

j. Provide education/guidance regarding situations where SDOH need to be addressed.

k. Serve as a liaison between care coordinators and SDOH entities 

l. Maintain knowledge and expertise in areas related to SDOH (benefits/entitlements, housing, economic stability, SNAP benefits, transportation, etc.)

Requirements

 Knowledge, Skills and Abilities Required 

• Excellent oral and written communication skills, including presenting and speaking in public settings

• Strong interpersonal and customer service skills including working with people in one on-one settings and group settings

• Ability to work independently, take initiative, multi-task, and prioritize work effectively

• Computer skills with proficiencies in Word, Excel, internet-based applications and the Microsoft operating system.

• Knowledge of health insurance programs and public coverage options. 

Preferred

• Bilingual (English and Spanish) and able to provide services in Spanish, both verbal and written. 

Credentials and Experience: 

Required

• Grade/Education Level: 12/High school diploma or equivalent Preferred

• Associate’s or bachelor’s degree in related field

• 1 year of experience working with programs and services intended for low-income, underserved populations.
