Ensure significant outreach and engagement of target population through building and sustaining strong community partnerships and networks. Moderate degree of travel in community.

ESSENTIAL JOB FUNCTIONS:

  • Conduct community outreach activities aimed at target population through the distribution of materials and information via presentations at local organizations including natural gathering sites, places of worship, and other community-based programs.
  • Assist with reaching program census goal
  • Assist in the coordination of billing with the Program Director
  • Assist the Program Director with ensuring that all billable activity has back up documentation
  • Report all outreach activity weekly to the Program Director
  • Identify new community partners and market upcoming events and community-based activities.
  • Support client satisfaction surveys to identify program strengths and areas to improve. Provide feedback to multidisciplinary team based on client observations and interactions.
  • Assist program participants with accessing needed community services (housing, medical, vocational, food, clothing, etc.) through network development, referral and follow up.
  • Actively participate as integral part of the PROS multidisciplinary team including family care planning and collaboration.
  • Perform basic accountability tasks, include compiling and maintaining all necessary records and assist administrative staff in collection of information for program effectiveness.
  • Participate in all trainings for the Whole Family Health Model.
  • And other duties as assigned

ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:

  • Working knowledge of computer software and electronic health record systems and other technology required by ICL.
  • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards and to prepare accurate and timely documentation, reports and other written material as assigned.
  • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model.
  • Knowledge of Medicaid, Social Security and other entitlements preferred.
  • Ability and willingness to regularly travel, in some instances with clients, to many locations using various modes of reliable and safe transportation
  • Ability to secure the cooperation of and work effectively with others
  • Ability to work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.
  • Basic understanding of the causes and processes of mental illness and substance abuse disorder.
  • Willingness to continually learn and apply knowledge and willingness to participate in in-service training and development activities.
  • Ability to function as an effective team member, including performing share of work, cooperating with coworkers, and securing cooperation, and maintaining professional relationships and boundaries with co-workers, Coordinators and consumers.
  • Ability to use sound judgment in identifying and solving problems, and knowing when to seek assistance.
  • Ability to be aware of self and one’s impact on others
  • Ability to learn, understand and comply with all regulations, policies and procedures.
  • Ability to organize time effectively to accomplish all tasks in a timely way and meet deadlines without prompting.

  • Ability to communicate effectively with staff, consumers/residents, families, and the public both orally and in writing.
  • Ability to prepare accurate and timely documentation, reports and other written material as assigned.
  • Ability to work independently, and to conform to all applicable safety and accountability measures

QUALIFICATIONS AND EXPERIENCE:

Minimum: Bachelor’s degree with at least four years’ related experience in the social service field.

Preferred: Master’s degree in a mental health field; supervisory experience.
