Outreach Coordinator - Help Me Grow

Job Summary:

The Outreach Coordinator will participate in outreach events, connect with community partners, and educate the community about the Help Me Grow Services, early identification, and ASQ screenings. The outreach coordinator will work collaboratively with the HMG Case Manger and Supervisor to promote services and connect families to resources within the assigned County. 

Essential Functions:

• Assist in linking families with appropriate services and resources for support.
• Provide presentations about Help Me Grow Program and lead all Help Me Grow outreach events.
• Responsible to account for all work-related activities and maintaining own work assignments with minimal direction or supervision.
• Effectively engages and collaborates with individuals from diverse cultural, socioeconomic, and social backgrounds in a respectful and inclusive manner.
• Responsible for maintaining a caseload of 15-20 active families in the assigned county.
• Promote, coordinate, and assist in monthly Help Me Grow Coalition Meeting
• Responsible to complete meeting minutes, assist with coordination of meeting agenda, planning and organizing. 
• Must be able to establish, develop, and maintain collaborative partnerships and networking resources to support the needs of the families both internally and externally.
• Accurately enters and maintains client and program data in the Apricot database; answers and routes telephone calls; and utilizes computer software and office technology to perform administrative and program support functions
• Maintain and submit required family documentation and data in a timely manner.
• Promote and administer ASQ assessments and screenings, child development milestones, and family support services through community presentations, outreach events, partner collaborations, and targeted communication campaigns. 
• Analyzes community needs, demographic trends, and stakeholder feedback within the assigned county to develop and implement outreach strategies, evaluate engagement efforts, and enhance service delivery to diverse populations. Responsible for maintaining a full understanding of EPU policies and procedures and maintaining proper confidentiality.
• Maintain, manage, and assist in processing records and reports to include filing and maintaining charts in a confidential manner that always meets HIPAA and other applicable confidentiality regulations.
• Required to hold a valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance for local travel.
• EPU facility hours are Monday – Friday, 8:00 AM –6:00 PM, with some exceptions based on family’s needs. Position will require ability to work evenings and weekends per business needs.
• Must perform other duties as they are assigned by the Supervisor, Director and/or President/Chief Executive Officer.

Knowledge, Skill, and Experience:

Education: Minimum of a High School Diploma or equivalent. Prefer associate’s degree or higher in social work, early childhood development, or other related field. Will consider actual work experience in lieu of degree. 

Skills: Must have strong communication skills, written and verbal, planning/organizing, information gathering, identifying solutions, summarization, listening comprehension, goal setting, multi-tasking, teamwork, and   

collaboration, initiative, and able to make independent decisions within guidelines and code of ethics. Must be self-motivated.

Experience:

• 2+ years’ experience working with families and young children
• Case management and a strong understanding of community resources required.
• Must work with a diverse population and be a team player.
• Must have the capacity to engage families and yet maintain the boundaries of a helping relationship. 
• Demonstrated ability to build positive relationships with children and families through compassionate, respectful, and culturally responsive interactions
• Strong communication and problem-solving skills, with the ability to engage, support, and empower families to identify solutions, overcome barriers, and advocate for their needs.
• Must hold a valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.
• Must have reliable transportation for local travel.
• Bi-lingual in Spanish is a plus.
• Computer literate, knowledge of Microsoft Office products.

Physical Demands:

Typical Working Condition: Staff are required to visit community settings and sometimes a the local EPU office location. EPU does not have control over the location and condition of all the environments staff may be exposed. 

Equipment Used: Personal vehicle for local travel to attend meetings in the local area, computers, printers, telephones, and fax machines.

Essential Physical Tasks: Working with children and their families require the ability to sit on floors, reach, twist, turn, kneel, stand, stoop, crawl, lift, carry, push and pull. Working in office setting with computers, telephones and other office related equipment.
